Although both cause penile curvature, congenital curvature and Peyronie’s Disease have distinct origins, making it important to identify each condition for an appropriate approach to correcting congenital curvature.

Many men spend much of their lives with penile curvature without knowing exactly what it means. Only later, when they feel insecurity or difficulty during sexual intercourse, do they begin to research the topic and find information that can generate concern.

The confusion is understandable. After all, congenital curvature and Peyronie’s Disease can have similar impacts on sexual life, but they are not the same condition.

A penile congenital curvature is stable and related to penile anatomy, while an acquired curvature, such as Peyronie’s Disease, originates from a fibrosis process. Distinguishing one situation from the other is important to define an appropriate approach, with safety and without compromising sexual quality of life.

What is congenital curvature?

Congenital penile curvature is an anatomical alteration of the penis present since birth. However, it is usually becomes evident during puberty, when the young man begins to have erections.

Unlike Peyronie’s Disease, congenital curvature is not caused by a fibrotic plaque.

How congenital curvature forms

In congenital curvature, there is a structural difference in the way penile tissues develop, potentially causing the penis to curve during erection.

The tunica albuginea – the structure that envelops the cavernous bodies and participates in erection rigidity – is formed with a natural asymmetry due to a difference in elasticity. When one side has less expansion compared to the other, the penis can curve toward the shorter side.

When signs usually appear

Many men recognize the curvature during adolescence or at the beginning of sexual life, when erections become more rigid and frequent.

As any penis can present a discrete natural curvature, the young man can consider that his anatomical variation is acceptable.

However, when initiating sexual life, this curvature can be associated with insecurity or difficulty in penetration, making it important to seek specialized evaluation.

This does not mean that all cases compromise intimate life, but a specialized evaluation is important to assess whether the curvature interferes with sexual function and whether the patient can benefit from treatment.

Does congenital curvature cause pain?

In general, congenital curvature does not cause pain. This is an important difference compared to the active phase of Peyronie’s Disease, in which there can be erection with pain.

In congenital curvature, the impact can be functional, with difficulty of penetration or discomfort in certain positions. There can also be emotional impact.

What differentiates congenital curvature from Peyronie’s Disease?

The main difference between Peyronie and congenital deformity is in the origin of the condition.

Congenital curvature is an anatomical alteration present since birth. Peyronie’s Disease is an acquired condition that arises over time.

Congenital curvature has no fibrotic plaque

In congenital curvature, there is no fibrotic plaque. There is also usually no palpable hardening, calcification or a localized area of fibrosis. The origin of the condition is structural, not acquired.

Peyronie is an acquired fibrotic disease

Peyronie’s Disease occurs when a plaque of fibrosis forms on the tunica albuginea, which causes loss of elasticity in a certain area of the penis.

With this, during erection, the affected region begins to present a progressive curvature and is subject to other deformities, such as narrowing and shortening. There can also be pain and erectile dysfunction.
