What is Peyronie’s disease and what are its symptoms?

Peyronie’s disease is characterised by the accumulation of small scars that form fibrous plaques inside the penis, which may cause abnormal curvature that is noticeable during erection and may make sexual intercourse difficult or even impossible.

The main causes are associated with microtrauma to the penis during intercourse, sports activities or falls. In addition, inflammation, genetic conditions, smoking, diabetes and hypertension may also be risk factors.

The most common symptoms include:

  • Pain during erection;
  • Palpable plaques or nodules in the penis;
  • Reduced ability to obtain or maintain an erection;
  • Erectile dysfunction;
  • Anxiety and stress related to sex.

When is penile curvature surgery indicated?

Peyronie’s disease surgery is indicated when penile curvature interferes with or prevents intercourse, causing pain or discomfort during sexual activity.

However, it is important to note that the surgical decision does not depend exclusively on the patient. Therefore, it is essential that a urologist specialised in Peyronie’s disease assesses the case to determine whether the surgical procedure is truly the best treatment option.

How much does Peyronie’s disease surgery cost?

Correction of Peyronie’s disease: main procedures

This is because specialists may perform penile curvature correction surgery using different techniques, such as grafts, tissue expansion or penile prosthesis implantation.

Penile reconstruction with or without grafts?

The benefits of graft surgery include preservation of erectile function and improvement of curvature. However, this method has disadvantages, such as the risk that the body may reabsorb or reject the graft.

Penile graft

A graft is tissue removed from a part of the body without its own blood supply. Although the technique may preserve erectile function and improve curvature, it has been falling out of use due to disadvantages such as:

  • It requires many suture lines that may be felt by the patient after healing;
  • As grafts have different sizes and thicknesses, the penis may develop a “bump effect” that is noticeable to the touch;
  • Because there are different sizes and thicknesses, the penile graft may have different levels of resistance, which increases the risk of rupture;
  • The graft has no blood supply, so the tissue tends to decrease in size.

Tissue expansion

Tissue expansion is a technique that consists of making incisions in the fibrotic area of the penis. The cuts help relax the tissues, reducing the tension caused by the curvature and making the penis straighter.

A study published in the Asian Journal of Andrology on penile lengthening techniques associated with penile prosthesis analysed 416 patients and showed an average gain of 3.3 centimetres in penile length, in addition to curvature correction.

In addition, the same publication also reports an incidence of haematomas of approximately 20% and an infection rate of only 0.24%. There were no records of glans necrosis.

Peyronie’s disease surgery: do I need a penile prosthesis?

Erectile dysfunction is a common problem in men with Peyronie’s disease. This happens because penile curvature caused by scar tissue accumulation produces changes in the internal structure of the penis, making the blood flow required for erection more difficult.

In this case, a penile prosthesis may be indicated because the implant helps restore penetration capacity.

As Peyronie’s disease is progressive, meaning that it may progress over time, penile prosthesis implantation is an alternative indicated in some cases to help restore sexual function.

In addition, another important point is that the progression of Peyronie’s disease may require surgical interventions at certain stages. In this context, the prosthesis may contribute to a more stable solution, according to medical assessment.

Malleable penile prosthesis implant

Malleable penile prosthesis implantation, also known as semi-rigid implantation, consists of placing flexible cylinders inside the corpora cavernosa.

The main characteristic of this type of prosthesis is that it keeps the penis elongated while allowing it to be accommodated flexibly in underwear. Therefore, to initiate intercourse, the man must position the penis upwards.

The Portuguese National Health Service (SNS ) may cover the procedure in specific cases, subject to strict clinical criteria.

Inflatable penile prosthesis implant

The inflatable penile prosthesis is a type of prosthesis composed of three components: a saline reservoir, a pump and two cylinders.

Its operation depends on activating the pump, which sends saline to the cylinders. This movement makes the penis become erect.

The value of surgery to correct a curved penis with an inflatable prosthesis varies according to the type of prosthesis, the hospital and the medical team. In most cases, the costs are borne by the patient, although support from health insurance may exist.
