At the time of surgery, penile reconstruction techniques may be associated to expand the tissues, which, in selected cases, may influence the sizing of the implanted device, within the anatomical and clinical limits of each patient.
Penile prosthesis and an increase in penis size are not directly related. The goal of a penile prosthesis is to restore the functional capacity of the penis, which may be compromised in cases of loss of rigidity.
In some cases of severe erectile dysfunction associated with Peyronie’s disease, placement of a penile prosthesis may be considered. Among the most frequent questions in consultation are those related to the final appearance of the penis after the procedure.
Below is general information about penile length after implantation, one of the aspects frequently discussed in consultation.
Does a penile prosthesis increase penis size? Understand the truth
The purpose of a penile prosthesis is to restore axial rigidity to the organ, making it functional again for sexual intercourse, according to the individual response and the clinical context. Therefore, a penile prosthesis is not intended to increase penis size.
In cases where penile fibrosis is associated with penile curvature and erectile dysfunction, penile prosthesis implantation may, in selected situations, be combined with penile reconstructive maneuvers. These approaches may aim to improve the anatomical conditions for implantation and seek to recover, as far as possible and within anatomical limits, dimensions previously compromised by fibrosis.
The approach is planned taking into account anatomical limits and the need to preserve relevant structures, such as the nerves and the urethra, while seeking to minimize the risks inherent to the procedure.
After this first stage of the surgical procedure, the surgeon performs the sizing of the penile prosthesis, selecting the size most appropriate for the reconstructed penis.
This does not mean that the penile prosthesis is intended to increase penis size. In some selected cases, prior reconstruction may allow better anatomical conditions for implantation, which may influence the sizing of the device within the anatomical limits of each patient.
Without associated reconstructive maneuvers, isolated penile prosthesis implantation does not, as a rule, aim to modify penile dimensions.

How many centimeters can the penis grow?
The final size and girth of the penis after penile reconstruction for Peyronie’s disease or erectile dysfunction varies according to anatomy, the degree of fibrosis, and the type of prosthesis.
In a study published by Dr. Paulo Egydio in 2020 in The Journal of Sexual Medicine, which evaluated 416 patients — 287 of whom had Peyronie’s disease — the reconstructive strategy associated with implantation allowed an intraoperative penile gain between 2 and 6 cm, with an average of 3.3 cm.
It is important to emphasize that it is not possible to estimate penile dimensions in advance. Only during surgery can the doctor assess the characteristics of the organ and determine whether the conditions are present to perform adjunctive reconstructive maneuvers.
How much does penile enlargement surgery cost?
A penile prosthesis is not indicated as penile enlargement surgery. In cases of shortening caused by penile fibrosis, associated reconstructive techniques may be considered, with the aim of recovering, as far as possible, penile dimensions within anatomical limits.
When discussing penile implant surgery, it is necessary to consider not only the cost of the prosthesis, but also the healthcare and technical expenses associated with the surgical act. Information about costs should be provided in an appropriate context, on an individualized basis, and according to the clinical and healthcare framework.
